To ensure that you receive the proper dental care, it’s important that we have your most up to date medication information. To ensure this, we ask that you do the following things when you arrive at our office:
- Inform us of any existing medical conditions that you may have.
- Bring an updated list of all current medications that you are taking.
- Take your time when completing your medical history form and carefully fill out all the relevant details.
- Bring your doctor’s contact information if you’re under a physician’s care. This allows us to consult them in the event you require a treatment that may affect your medical condition.
Please note that some medical conditions may rule out certain dental work from being performed until your physician has cleared that it’s safe to do so. By being able to communicate with your doctor, we are able to consult with them to determine what is best for your case.
